CATCH Houston Wins Dual Awards in Federal Home-Based Care Challenge

Houston – October 06, 2026 -- CATCH Greater Houston has been named a Phase 1 winner in two of the ten awarded tracks of the Health at Home Challenge, a national competition run by the Administration for Community Living (ACL) within the U.S. Department of Health and Human Services.

CATCH secures dual recognition in federal care coordination competition

The nonprofit won as a community care hub within a national consortium led by the Oregon Wellness Network, spanning eight hubs across eight states, and separately as the designated lead of the Texas Hub Network, a statewide consortium formed with the Texas Health and Human Services Commission.

Challenge targets up to one million dually eligible beneficiaries nationwide

The competition aims to improve health outcomes and reduce costs for Medicare and Medicaid beneficiaries who are dually eligible or near-dually eligible — older adults and people with disabilities managing multiple chronic conditions and at least one functional limitation. ACL has set a goal of reaching up to one million such beneficiaries nationally through advanced community care networks.

Both teams advance to year-long implementation phase through July 2027

CATCH's two winning teams progress to Phase 2, the Implementation Accelerator, running from August 2026 through July 2027. ACL will select up to five Phase 2 winners to advance to Phase 3 scaling awards beginning August 2027.
