Karyopharm Lands Two ASH 2026 Orals for Selinexor in Myelofibrosis

Newton, Mass. – November 04, 2026 -- Karyopharm Therapeutics (Nasdaq: KPTI) will present Week 48 data from its Phase 3 SENTRY trial and early monotherapy data from its Phase 2 SENTRY-2 trial in two oral sessions at ASH 2026, giving selinexor two oral slots in myelofibrosis. A third abstract, from the STOMP trial in multiple myeloma, was selected for a poster.

SENTRY Week 48 data go on stage December 13

The SENTRY presentation (publication number 526) is scheduled for December 13, 2026, from 10:15 a.m. to 10:30 a.m. CT. It will cover updated results for selinexor plus ruxolitinib in JAK inhibitor-naïve myelofibrosis. Chief Medical Officer Reshma Rangwala said the data show ongoing benefit through week 48.

SENTRY enrolled 353 patients with platelet counts above 100 x 109/L. They were randomized 2-to-1 to once-weekly 60 mg selinexor plus ruxolitinib or placebo plus ruxolitinib. The co-primary endpoints are spleen volume reduction of at least 35% (SVR35) at week 24 and the average change in absolute total symptom score over 24 weeks.

The trial results were previously presented at the 2026 ASCO Annual Meeting and published in the Journal of Clinical Oncology. At the 2026 European Hematology Association Congress, the presentation was named one of the meeting's six best abstracts.

SENTRY-2 tests selinexor monotherapy at 60 mg in 29 patients per arm

The SENTRY-2 oral presentation (publication number 244) is set for December 12, 2026, from 2:45 p.m. to 3:00 p.m. CT. Karyopharm says data from the 60 mg cohort support monotherapy activity in JAK inhibitor-naïve myelofibrosis.

The multicenter, open-label study enrolls patients with thrombocytopenia (platelet count of at least 50 x 109/L). Cohort 1 receives 60 mg once weekly, with 29 patients per arm. Patients whose spleen volume reduction is below 10% at Week 12 or below 35% at Week 24 can add ruxolitinib, pacritinib or momelotinib, depending on platelet count.

The primary endpoint is SVR35 at Week 24. Key secondary endpoints are safety and absolute total symptom score at Week 24. Exploratory endpoints include changes in variant allele frequency and bone marrow fibrosis.

Myelofibrosis affects about 39,000 patients in the U.S. and EU, with JAK inhibitors the only approved class

Myelofibrosis affects roughly 20,000 patients in the United States and 19,000 in the European Union, according to Clarivate/DRG (2023). JAK inhibitors, including ruxolitinib, are the only approved class of therapies for the disease.

STOMP Arm 12 poster covers all-oral selinexor-mezigdomide combination in myeloma

The poster (publication number 2242) will be presented December 12, 2026, from 5:30 p.m. to 7:30 p.m. CT. It reports updated results for selinexor, mezigdomide and dexamethasone (SMd) in relapsed/refractory multiple myeloma patients who relapsed after or are ineligible for T-cell-redirecting therapy.

Arm 12 enrolls adults with triple-class exposed disease and at least two prior lines of therapy. Phase 1 escalated selinexor from 40 mg once weekly to 60 mg on days 1, 8 and 15 of a 28-day cycle. Mezigdomide is given at 0.6 or 1.0 mg daily on days 1-21, and dexamethasone at 40 mg weekly. Phase 2 primary objectives are objective response rate and safety at the recommended Phase 2 dose.
