Noom CEO Proposes 'Habit Window' Framework to Fix GLP-1 Dropout Rates

Toronto – September 15, 2026 -- A new viewpoint paper published in the Journal of Medical Internet Research argues that GLP-1 weight-loss drugs fail long-term without paired digital behavioral coaching, citing high patient dropout and rapid weight regain as persistent industry problems. The paper, authored by Noom CEO Geoff Cook, introduces a "habit window hypothesis" and calls for a formal research agenda to test whether digital health apps can extend the durability of pharmacological weight loss.

The viewpoint, titled "Digital Behavioral Infrastructure for Glucagon-Like Peptide-1 Pharmacotherapy: Viewpoint on Persistence, Tolerability, and Postcessation Durability," maps medication persistence, gastrointestinal side-effect management, and postcessation weight regain as the three biggest commercial and clinical hurdles facing GLP-1 manufacturers and prescribers. It applies social cognitive theory and behavioral economics to argue these gaps require structural digital support rather than drug reformulation alone.

Cook proposes a pharmacologically defined 'habit window' for behavior change

Cook's hypothesis holds that GLP-1 drugs reduce constant food cravings, or "food noise," temporarily freeing mental capacity that patients can redirect toward building sustainable eating and exercise routines. He links this to a "Fresh Start" effect, suggesting the biological window created by the medication is finite and must be used before appetite cues return to baseline after treatment ends.

Noom says its GLP-1 Companion app already applies the paper's framework

Cook states that features described in the paper -- personalized medication reminders, interactive symptom management, and structured skill-building -- are already deployed in Noom's GLP-1 Companion product, built around dual-process "fast and slow thinking" behavioral models. "The objective becomes building healthy behaviors while the habit window is pharmacologically open," Cook said, noting the features are operational rather than theoretical.

Noom commits to randomized controlled trials to validate the hypothesis

The paper states that early observational data is promising but insufficient, calling for prospective randomized controlled trials to test the habit window framework rigorously. Cook said Noom will fund RCTs aligned with the research agenda outlined in the paper's Table 1, targeting the specific mechanisms and interventions raised in the viewpoint.
