UPMC Expands AI Transitional Care Model to 18 Hospitals

United States – October 17, 2026 -- UPMC will expand its AI-native transitional care management model from UPMC Jameson to 18 hospitals this fall, working with Andor Health and Psynergy Health.

Every enrolled patient receives outreach within 48 hours of discharge

Andor Health's ThinkAndor platform uses AI to automate initial outreach, analyze clinical information, prioritize emerging needs and prepare clinicians for patient encounters. Patients who need additional support can be escalated to a care coordinator.

Across transitional care workflows, Andor reports a 63% reduction in clinician time spent on non-clinical tasks per encounter and an 82% reduction in non-actionable monitoring alerts.

Psynergy Health and UPMC care teams supply the clinical layer

Clinical services from Psynergy Health and UPMC's care teams work alongside ThinkAndor. The model covers outreach, scheduling, clinical encounters, documentation and ongoing care coordination. AI handles automatable work, while clinicians focus on cases that require their judgment.

Partners refined the program for roughly 18 months before the rollout

UPMC, Andor Health and Psynergy Health adjusted the program based on real-world experience. Changes included strengthening patient education, moving automated check-ins earlier and co-branding outreach to reinforce the connection with UPMC.

The partners say the model can help health systems reduce avoidable readmissions, address referral leakage, expand clinical capacity and strengthen continuity after discharge. They frame the approach around accountability for patient outcomes rather than technology deployment alone.
